Practice Policies

Policies requiring patient signature are marked with an asterisk (*). Reviewing and signing these forms before receiving care is required to protect you, your provider, and Unscripted Medicine. These forms will be provided electronically when you schedule your initial consult, and must be signed at least 24 hours prior to your appointment or your appointment will be cancelled. You will be allowed to reschedule once the forms are complete.
